วัยรุ่น (Adolescence) คือช่วงอายุ 10–21 ปี เป็นช่วงที่มีการเปลี่ยนแปลงอย่างมากและรวดเร็วทั้งทางร่างกายและจิตใจ
Physical Growth
- Growth spurt กินเวลาประมาณ 2–4 ปี ควบคุมหลักโดย growth hormone
- ในช่วง growth spurt วัยรุ่นจะสะสมน้ำหนักได้ประมาณ 50% ของน้ำหนักผู้ใหญ่ที่เหมาะสม และความสูงเพิ่มขึ้น 25% ของความสูงผู้ใหญ่ที่เหมาะสม
- เด็กหญิง มี growth spurt เร็วกว่าเด็กชาย 18–24 เดือน
Endocrinologic Changes
การเปลี่ยนแปลงฮอร์โมนแบ่งเป็น 2 ระยะ:

Adrenarche — การเริ่มสร้าง adrenal androgen จากต่อมหมวกไต เกิดขึ้น ก่อน การ mature ของ hypothalamic-pituitary-gonadal (HPG) axis ประมาณ 2 ปี

True puberty — เกิดขึ้นเมื่อ gonadotropins (LH, FSH) และ gonadal sex steroids (estrogen, testosterone) เพิ่มสูงขึ้น โดยมีกลไก HPG axis ดังนี้:
- Hypothalamus → หลั่ง GnRH → Anterior pituitary → หลั่ง LH + FSH → Gonads → ผลิต sex steroids → Negative feedback กลับไปยัง hypothalamus และ pituitary
Physical Changes in Puberty
เพศชาย:
- Puberty เริ่มหลังเพศหญิง
- First sign of puberty = Testicular enlargement + scrotum บางลง ที่อายุ 11–12 ปี
- Facial hair และ axillary hair เกิดประมาณ 2 ปี หลังจาก pubic hair เริ่มขึ้น
เพศหญิง:
- Puberty เริ่มด้วย Thelarche (breast buds) เฉลี่ยอายุ 9.5 ปี
- Pubic hair ตามหลัง thelarche
- Menarche (ประจำเดือนครั้งแรก) เฉลี่ยอายุ 12.5 ปี หรือ 2–3 ปีหลัง thelarche
Tanner Staging (Sexual Maturity Ratings)
เพศชาย — Genitalia & Pubic Hair:

เพศหญิง — Breast:
